Service agreement & liability waiver

This example waiver shows how Example Pet Co. would collect consent during signup. It is sample text, not legal advice.

1. Care. Staff follow the care notes you provide and contact you if anything seems wrong.

2. Emergencies. If your pet needs urgent care and we can't reach you, we may take them to your listed vet.

3. Vaccines. Pets must be current on Rabies, DHPP and Bordetella.

4. Keys and access. Access details are stored securely and used only for booked visits.
